Hospice Care
Hospice is a specialized care that focuses on supporting you and your loved ones during advanced illness. The emphasis is on comfort and quality of life, rather than finding a cure. Hospice care enables you to live each day to the fullest as comfortably and dignified as possible.
Dedicated Hospice Registered Nurse (RN)
Hospice patients will receive regular visits from a hospice RN who will oversee their plan of care, with consultations by a hospice physician as needed.
Pain Management
Hospice care provides compassionate care to alleviate pain and improve comfort during end-of-life, including medications and supplies.
Care Beyond the Patient
Hospice care includes emotional and spiritual support for both the patient and their family during this phase of life. This includes family counseling during care and bereavement counseling for the family after the patient has passed.
Coordinated Care
Citadel will coordinate the patient’s care and medications across all of the patient’s medical providers, including the patient’s own doctors, hospice doctors, hospice nurses, hospice aides and all nursing home staff.
